Heart is completely situational item. Even on heros like Centaur (I would get bkb, blademail, pipe, armor before it), AM (only good against a lot of magic damge and burst), and CK (bkb, silvers edge) there are alot of items much better than heart.
But I don't think Heart needs a buff. It should be left as a situational item like many other items. It can still cripple maybe burst/magic damge based heroes
