Why do you want to ban him? If it has happened only 1 time maybe the other team decided to feed him for some reason.
If it has happened many times it's ok to report, but 1 time? I have made 105/0 on Tinker once in a smurf account just by fountain farming.

As a few people have pointed out already, it's only ever happened 1 time. That's not enough evidence.

did it in captain draft purposely to lessen the chance of player queue there,
less player queue = more chance he can make his antimage played in the same game as him

If there are other games where he is suspected of cheating then send a report with the evidence.

Otherwise people do play different modes and on different servers, guessing his intent through 1 game is not sufficient evidence.
I played on Australia server once before as well. (I lost, I played badly and the lag was not helpful)
